This survey has several questions about your current feelings about returning to regular church activities at All Nations when the local government lifts stay-at-home orders and guidance against public gatherings. We sincerely appreciate our church family taking the time to participate in this decision making through your contributions to this survey. Please thoughtfully and prayerfully complete this survey by May 28th.

Worship Service Re-Entry

When our officials open our city and county up to allow churches to meet again, which best describes your attitude toward returning to a worship service at church?

Which of the following describes the attendance level at which you would be comfortable attending a worship service?

Precautionary Measures

What precautions, if any, do you plan to take when you return to a worship service? I will wear a facemask I will stand and sit at least 6 feet apart from everyone else (excluding my own family) I will avoid shaking hands and other physical activity. I will avoid sharing crowded hallways with others. I will bring hand sanitizer. Not sure I'll do what the cdc recomends
